Care Coordination

Every client's needs are different, which is why we provide dedicated Care Coordination to coordinate support around each person's individual goals and circumstances. Our Care Coordinator gets to know our clients personally, building a clear picture of their preferences, routines and support networks so services can be organised in a way that genuinely fits their life.

Beyond coordination, our Care Coordinator keeps a close eye on how each client is going, monitoring wellbeing, identifying changes early, and making sure the right people are informed when circumstances shift. This ongoing oversight means our clients and their families always have a consistent, familiar point of contact who understands their care journey.

Natalia Santos is passionate about helping older Australians and their families feel supported throughout their aged care journey. As an Aged Care Division Coordinator, she enjoys building strong relationships, assisting clients with their needs, and ensuring every detail is managed with care. Known for her friendly approach, strong organisational skills, and attention to detail, Natalia takes pride in creating positive experiences for both clients and colleagues.
