End of Life Care
How to access the End-of-Life Pathway

Staying at home can offer greater dignity, privacy and a sense of control during a deeply personal time. It often allows for more meaningful moments with loved ones, reduced stress from hospital environments, and care that is tailored to individual wishes and cultural needs. The End-of-Life Pathway under Support at Home provides dedicated short-term funding and support so eligible people can remain at home in their final months of life, with access to the clinical, personal and practical care they need.
