What is Care Coordination?
Care coordination is at the heart of how we support older Australians to live safely, independently and with dignity at home. For clients receiving Fully Coordinated Support at Home services, we provide a consistent point of contact through our Case Managers, who take the time to understand each client as an individual: their needs, goals, preferences, routines and existing support networks. From there, we coordinate services around what matters most to them, ensuring support is delivered safely, reliably and respectfully by appropriately qualified staff.
Beyond delivering services, our role is to stay closely engaged with how each client is experiencing their care. Our Case Managers monitor wellbeing and service outcomes over time, watching for changes in circumstances, whether that's a shift in mobility, health, living arrangements or personal goals, and responding early. In partnership with Trilogy Care, any changes we identify are clearly documented and communicated so they can be properly considered within the client's formal care plan. This connected approach means clients always have someone in their corner, ensuring the right people are informed, the right information is shared, and any need for change is picked up and acted on at the right time.
Our commitment is simple: care coordinated around the person, built on trust, transparency and continuity, so clients and their families can feel confident and supported every step of the way.
